BACKGROUND QUESTIONS
When you register for the New Mexico Teacher Assessments, you will be asked to answer the following background questions. Providing accurate and complete background information about yourself is important; the information that you provide will be kept confidential.
Background information is collected differently based on your method of registration.
- Registration on the Internet: Answer the questions interactively as you register.
- Registration by telephone: You will be asked by the operator to answer questions 10 and 11, below.
- Registration by U.S. mail: Enter your responses on the registration form. Use the following information to guide you in completing the registration form. The question number corresponds to the section of the registration form where you will record your response.
Current Educational Status
What is the highest educational level you have attained to date?
1. Freshman
2. Sophomore
3. Junior
4. Senior
5. Received bachelor's degree
6. Received master's degree
7. Received doctorate
New Mexico College or University Information
Indicate the New Mexico college or university that granted or will grant you a degree by selecting the two-digit code from "Institution Codes." In most cases, this will be the institution that granted your most recent or advanced degree.
If the institution that granted or will grant you a degree is the University of Phoenix, Wayland Baptist University, or Western Governors University, indicate this by selecting the appropriate two-digit code from "Institution Codes." If the college or university that granted or will grant you a degree is another out-of-state institution, proceed to question 10.
If you attended more than one institution listed, please select the code for the one that granted or will grant the most advanced degree.
If the New Mexico institution you attended is not listed, fill in 99 for the institution code and proceed to question 10.
Educator Preparation Program
Indicate the New Mexico educator preparation institution or alternative route through which you have completed your program or in which you are currently enrolled by selecting the two-digit code from "Institution Codes." This may be the same institution that you have indicated in your response to question 9.
If you received your educator preparation through a New Mexico alternative educator preparation program, proceed to question 11.
If you received your educator preparation through the University of Phoenix, Wayland Baptist University, or Western Governors University, indicate this by selecting the appropriate two-digit code from "Institution Codes." If you received your educator preparation through another out-of-state institution, proceed to question 12.
Your test scores will be released to the educator preparation program you indicate in your response to question 10 unless you include written notification that you do not wish your scores to be released to that institution. The purpose of providing institutions with test results is to give the institutions an opportunity to review their educator preparation programs in light of examinee performance and to make any modifications or changes to the programs that might be indicated by the test data.
Alternative Program
Indicate the New Mexico alternative educator preparation program where you have completed your program or are currently enrolled by selecting the two-digit code from "Institution Codes."
Your test scores will be released to the educator preparation program you indicate in your response to question 11 unless you include written notification that you do not wish your scores to be released to that institution. The purpose of providing institutions with test results is to give the institutions an opportunity to review their educator preparation programs in light of examinee performance and to make any modifications or changes to the programs that might be indicated by the test data.
Out-of-State Educator Preparation
If you have completed your educator preparation or are currently enrolled at a college or university outside of New Mexico, select the postal code that indicates the state in which this college or university is located. In many cases, this may be the same institution where you earned the degree that qualifies you to apply for licensure.
If you completed your educator preparation or are currently enrolled at the University of Phoenix,
Wayland Baptist University, or Western Governors University, answer questions 9 and/or 10 as applicable;
do not answer question 12.
